It is our policy to accept into our practice patients who agree to receive those immunizations recommended by the Centers for Disease Control.
We currently accept most major insurance plans. Your insurance information must be presented at the time of your visit so that we may file your claim. Our insurance office will verify coverage at your visit. All co-pays and deductibles are due at the time of service and may be paid by cash, check, Mastercard or Visa. If no insurance information is presented, we ask that you pay in full at the time of service.
If you cannot keep a scheduled appointment, we ask that you contact our office at least 24 hours in advance. We will be glad to reschedule your child’s appointment. This will allow others the opportunity to utilize this time. Patients who miss appointments without notifying our office in advance may be subject to dismissal.
For the convenience of our patients, we will gladly complete most health and medication forms free of charge. However, we do charge a minimal fee for those forms such as FMLA and most government forms requiring a longer time to complete. Please inquire about the charge prior to leaving any form.
